Ordering forms is easy.

Choose the catalog link you wish to order from. When you enter the catalog choose the type of form you wish to order and click on the "buy now" button to add the items to your shopping cart. When you are finished adding items to your shopping cart click on the "Next" button to add your address and shipping info. Once you type in your shipping address, click on the "Next" button again, and you will see your order computed to include tax and shipping information. If everything is correct, click on "Checkout" to enter the secure ordering area. Select either "Mailing or Faxing Payment" or "On Account" under Payment Options. If you have any special instructions, enter them in the box provided. When ready, click "Place Order" and your order will immediately be e-mailed to us and you will be sent a confirmation to your own e-mail address.

Order totals 25 items or less, shipping costs $7.75
Order totals 26 to 50 items, shipping costs $9.00
Order totals 51 to 300 items, shipping costs $11.00
Order totals 301 to 500 items, shipping costs $13.00
Order totals more than 500 items, shipping costs $15.00
